Use legs: Make sure you use your legs when lifting to avoid stress on your back. Sleeping position is also important for chronic back problems. Finally, some short term physical therapy where they can teach you a home back program to strengthen your back muscles and improve your range of motion is very helpful. Just don't overdo it too soon.

See details: Avoid the actions that you know will cause a problem. Do exercises to improve core strength and flexibility, lift with your legs and not your back, do not try to lift heavier objects than you can handle, etc etc.
