How do you treat a subluxation of a jaw condyle, its still in the joint but has moved some. mouth guards have been tried with no success?

Oral surgeon: See an oral surgeon for advice. He/she may refer you to a TMJ expert. Any dentist can be a TMJ expert with the proper training and experience. Most commonly, oral surgeons, prosthodontists, and orofacial pain specialists. Ask your MD, your dentist and your dental society for referrals.
