My mri results came back saying: mild trochlear dysphasia with shallow trochlear depth, which can predispose to patellar instability. patellar chondromalacia with a full thick lness chondral defect at the genu of the patella. what does this mean? surgery

Not necessarily: So there are findings on the MRI consistent with patellafemoral instability as well as damage to the undersurface of the patella. Non operative options include therapy or injections and bracing, surgery may be an option of you fail nonoperative care.
