TMJ: Tmj can cause a range of symptoms and be caused by multiple factors, mostly functional which result in strain on muscles and ligaments. This in turn can affect the adjacent areas (ears, neck, headaches). You need to have the condition evaluated by a specialist in tmj. Stopping the cause( if possible) would be great but not always possible, but stopping the affects can be treated.

Yes & No: Tmj (joint) is anatomy - everyone has 2 of them, one on each side. Tmd (disorder) is a problem with one or both joints. Stress will make tmd worsen & the tension in your neck indicates you have more stress. It's good that the ear issues have improved. You need to address the stress - perhaps exercise or ginding time every day to destress will help.

Not necessarily: Evaluate how myou sleep. Try pillows designed this way supporting your neck. This will help with TMJ as well.
