Possible nonunion: with the good response for the first few months and the the recurrence of the pain, one needs to evaluate for a jinx join and failure of the graft or fixations Dan workout would include X-rays and CT Scan, also one would have to question wether additional pathology was not addressed and additional levels have now created an issue and an MRI would be of benefit as well

Difficult problem : There are many causes for pain post op fusion. Sometimes the surgical prosthesis loosens. Many times it is the discs above and below the fusion levels that give a problem. Less likely infection based on timeline. Flexing and extended X-ray is important as is MRI to look at the discs. In the neck scars are not as much a problem. Speak with your surgeon for your options.
