If white spots on tonsils do not go away with amoxicillin, could it be something other than strep?

Yes: It could be viral like mononucleosis

Not likely strep: This doesn't sound like strep. Unless you have fever, new sore throat and positive culture, antibiotics are usually avoided. Instead, it may be chronic tonsillitis as others have suggested. Try gargling with baking soda and salt dissolved in water. If the spots persist, seek the care of a local ENT for evaluation.
