Mallet toe.: Swelling with bruising in the area of the tip of the second toenail usually means trauma. Perhaps not the gross trauma, like kicking something, but the subtle low grade type. Many times the second toe is longer than the first. The extra length puts the tip of the toe in a position easily hurt by shoes, socks or even lightly bumping something repeatedly. Not much to do other than protecting it.
