Speak with dentist: All instruments are either autoclavable or disposable. Spore tests are done in the office on a daily basis to check the autoclave. All instruments which are autoclaved are in a autoclave bag with a sensor that changes color denoting the instruments are sterile. All autoclave bags are usually opened up in front of the patient.

Sterilization: All dentists in the United States are required by law to correctly sterilize their instruments between patients. If you are unsure nicely ask the dentist how they sterilize their instruments. Old does not mean unsterile. If you do not receive a satisfactory answer then leave, find a different dentist and report first dentist to Office if Professional Discipline.
