Could be infection: Diabetics who get a perirectal infection can get into serious trouble very quickly. He needs to see a doctor today to make sure that either, a) he doesn't have an infection, or b) if he has one, it gets treated quickly. Sometimes an urgent operation is necessary to prevent or treat worse problems like sepsis or death.
