Yes. Infected: what you described sounds like an infection. It may be early in its course. THe wound should be cleaned with Epsom salt and peroxide. Antibiotic cream (NOT ointment) should be used on the wound and it should be kept covered. HE should consult a podiatrist because he may need antibiotics.

Get toe the doctor.: Without even knowing the particulars of his general health and is success in controlling his sugar, there are concerns. A swollen toe that when pressed exudes water makes me think that he may have cellulitis. Cellulitis in the toe is bad enough but in a diabetic's, it could become a problem. Quit fooling around with this and get to whoever controls his diabetes ASAP.
