No single best: While I speak for other parts of the world, in the States, there is no single best toothpaste, not even one w/your specific requirements. Go see your Dentist to treat your cavities then ask him/her which toothpaste recommended.

Toothpaste: Toothpaste does not cure cavities, nor does it lessen pain. Only a Dentist can do that for you. The Dentist can also whiten the teeth after the cavities (infections) have been eliminated and recommend the best toothpaste for you. Whitening toothpastes tend to be more abrasive and can therefore do damage over time. You must see Dentist ASAP before you have no teeth.
