Many possibilities: Pain in an extremity after an IV is usually related to bleeding or bruising under the skin, that may not even be visible. But sometimes it can be a sign of infection. A clot in the vein is common, but doesn't usually require treatment unless it's infected or is associated with significant swelling of the arm.
