No, but it happens.: No, the majority of patients do not need blood transfusion after hip replacement. Every patient is a little different, and there can be multiple reasons for doing a blood transfusion. Aside from replacing intraoperative blood losses, if you started the operation with a low blood count, having a blood transfusion may lower the risk of certain cardiac complications as you recover.

Blood transfusion: For primary (first time) hip replacement surgery it is not common that patients need blood transfusions. It's important to have your blood level checked beforehand and if it is low figure out why it is low. Best predictor of needing a blood transfusion is adequate blood levels beforehand. Blood levels can be optimized preop with intravenous iron and/or things like epogen (epoetin alfa).
