Thoughts: The gp almost certainly suspects strep throat. If the following 4 criteria are met, then there is 38-63% likelihood: 1) no cough, 2) fever, 3) tonsillar exudate and 4) tender lymph nodes in front of neck. This is not perfect, so in the US it is recommended that testing be done to confirm. Waiting for culture results to initiate antibiotics does not increase risk of rheumatic fever.
