Go to the ER: If your pain is that bad, then it's possible that you could have an ovarian torsion from a cyst/growth on the ovary that twists over on itselfs and constricts the blood supply to ovary, leading to necrosis/cell death. If the pain is that bad and has sudden onset, that comes and goes, either see your GYN right away or go to the ED for an ultrasound.

Exam needed: Your symptoms are nondiagnostic and more information and an examination are needed for diagnosis,advice and treatment. If the discomfort is severe,recurrent and in the pelvic region, an examination by a gynecologist is urgently needed and the appropriate tests will be ordered.
