Epidural: Well, there was a manufacturer in Massachusetts that was selling an injectable for epidurals that was contaminated with fungus..this lead to fungal meningitis and death in several people. Any infection, bacterial or otherwise is also a complication. Bone infection the most notable. Worsening pain at the site is also a possible side effect.

Epidural is safe: Since the spinal cord ends at L1, L2 level, there is minimal chance of injury to the spinal chord during a lumbar epidural steroid injection, especially when done under flourscopy. Hope that helps.
