The vast majority: of all calcifications on mammography are benign and require no follow up or biopsy. When calcifications look suspicious enough to biopsy, 75% or so turn out benign. Radiologists evaluate calcifications based on shape, size, and distribution. Many are so typically benign that radiologist's don't bother mentioning them in the report.
