I take 25mg losartan which keeps bp @ 112/67 @ home but get 150-160/100 @ dr. appt. does this mean it's not effective for white coat htn?

No: Your medicine is effective. By definition, white coat hypertension is BP that's normal at home, but elevated with the stress of a doctor visit. No matter what medicine you take, when you're stressed (like walking up a hill), your BP's going to rise. That's normal. (physiologic).
