Toe extensor tendon: The big toe extensor tendon runs along the top of the foot and crosses the metatarsals between 1st and 2nd toe. The fact that the pain is exacerbated with dynamic motion more distally at the toe area suggests that at least some of the pain is coming the toe extensor tendon. Persistent pain may warrant a thorough evaluation by a foot and ankle specialist.
