Impingement syndrome: You are describing the radiologic findings of impingement syndrome. Basically arthritic changes at the joint between the acromion and clavicle can cause a bone spur to form that pushes down on the rotator cuff muscles ( including the supraspinatous muscle); causing inflammation, fraying and potentially tear. This causes pain when raising the arm or internally rotating it to reach behind the back
