High creatinine: Are you on dialysis? That creatinine suggests almost complete lack of kidney function. With back pain there could be obstruction to both kidneys. I would go right to the emergency room for an ultrasound of your kidneys and work up. If you are not on dialysis you may need to be. If you are obstructed relief of the obstruction could be curative.
