i've tongue twitches every days since 1.5 month. normal neuro exam and clean emg 1 month after the onset. i'm a 28 y-o man. could it be bulbar als ?

Can't tell: your history is too short. If your neurological exam is normal and twitching of the tongue is the only symptom then nothing more needs to be done at this time. Twitching of voluntary muscles is very common though it is rather uncommon in the tongue. I would recommend a follow up history and exam in 3 months.

Movement disorder: If you had tongue weakness with fasciculations, that might be worrisome, but your twitches are far more benign. These kinds of movements might be seen from medication, such as tardive dyskinesia, or even secondary to dental issues.
