I am a 41-year-old woman with severe health anxiety. normal clinical exam by neurologist and spine surgeon : clean emg of left arm. now experiencing twitching in calves . could i still have als? extreme health anxiety?

It is not feasible to provide a meaningful opinion without taking additional history, physical examination and may be some tests. It would be prudent to see your doctor and discuss if you may be depressed. Counter-intuitive as it may seem, exercise may help. Start with low intensity exercise such as walking and gradually increase the intensity as tolerated. Drink more water. Wish you good health!

Your calf twitching could represent benign fasciculations, secondary to muscle membrane irritability. You are young to encounter ALS. Suggest you deal with your anxiety by enrolling in cognitive-behavioural therapy and/or biofeedback.
