Pain under rib cage: please provide more information in regards to the pain, is it dull, throbbing, stabbing or burning in nature. As you have stated that you are a smoker. and if it burning sensation under your rib cage then it could be due to acid reflux take zantac (ranitidine) and see if that helps alleviate the pain

Need more info...: Unless X-ray was recent enough to be associated with the pain you are currently experiencing, then all possibilities regarding workup remain on the table--including repeat imaging. The first step is a good physical exam and history gathering. If you have associated shortness of breath, this requires an immediate evaluation.

Many causes: Gall bladder may be cause. If clinically indicated gall bladder evaluation with ultrasound and HIDA exam with cck will evaluate gb function. Sometimes abnormalities of liver and pancreas have to be excluded. Musculoskeletal causes of rib pain common.Changes in lungs can cause this pain.Having a chest xray one year ago does not exclude problem.You should see physician for exam and appropriate tests
