Neither: Extensive research studies demonstrate inferiority of aspirin in any dose lower than 325 mg four times daily, which can cause profound gastritis. In my opinion, the Aggrenox combination with low dose aspirin is most useful, followed by clopidigrel.

Probably: There is no definitive answer. A small % of the population is aspirin resistant - a blood test can determine if you are. Higher doses are needed. A small % of people will get stomach upset or internal bleeding - the lower dose should be used. For the majority, I recommend 81 mg but some of my colleagues disagree and prefer 325 mg.
