HOCM: If there's a family incidence of sudden death or if your heart is very thick we generally recommend an icd. Likewise if theres a history of syncope or serious arrhythmia. Discuss the reasons for the recommendation with your doctor who knows you and your situation.

May be: There are specific criteria for needing a icd for hocm. If you have a fairly thick hypertrophied heart muscle, history of rhythm problems or a family history. Not everybody needs one if your gradient is not that high. You can easily get the guidelines from the acc/ aha website of cardiospurce and discuss with your doctor and ask him why you are at a higher risk of arrhythmias.
