Yes.: A very high TSH suggests long-term underactive thyroid. T4, T3 (liothyronine) and thyroid antibody tests are appropriate. Ultrasound might not be necessary.

Ultrasound: You don't really need any other lab, but would benefit from an ultrasound, particularly if your doctor thinks your thyroid feels abnormal. You need to receive brand name T4 at a dose that returns your TSH to normal.
