Several possible: A little blood can look like a lot (!) when it mixes with the water in the toile. Bright red blood implies bleeding from the lower end of your digestive tract, including the rectum or anus. Hemorrhoids? Anal fissure? Have you been constipated lately? I can't think how heart failure would cause this directly. See your doctor right away.
