Formal bx: What were symptoms requiring EBUS and why wasnt bx performed at that time. Mediastinoscopy with nodal dissection could be very helpful. Necortic paratracheal node could be TB, sarcoid or necrotic lymphoma. Treatment if necesaary depends on histologic dx.

See below: A lymph node that large is very concerning for cancer. It could be TB. Sarcoidosis is another potential cause. Those 3 would be my top differential with cancer being #1, sarcoid #2 and TB #3.
