Several things: There are many causes for bad breath. Among the more common are gingivitis (inflammed or infected gums), possibly from poor brushing and flossing, and little stones that are formed in the holes in your tonsils, called tonsilloliths. There are more rare causes too. If good brushing and flossing doesn't help, you might see your dentist, primary doctor or nurse practitioner for help.

Anaerobic bacteria: Bad breath is caused by billions of bacteria in your mouth, the same bacteria also may cause gum desease. The reason it smells is bacause those bacteria produce gases and other waste in your mouth, and when you exhale, people around you may smell this very characteristic smell.

Poor oral hygiene!: While acid reflux can be an issue, it is no longer considered a primary cause of bad breath. The major causes of non-food related breath odors (garlic etc.) are gum disease and rotting food particles on the tongue. Floss daily ; use a toothbrush w-toothpaste to brush as far back on your tongue as possible. (colgate total with antibacterial triclosan works great!) voila, no more bad breath!

Bacteria: The most common cause of bad breath is the bacteria that live between the papillae of the tongue. Stomach disorders can also add nasty odors to the mix as well as the bacteria that cause gum disease.

Many causes: Bad breath can be caused by various conditions. Food which begins breakdown in the mouth eventually is broken down enters the bloodstream carried to your lungs and finally given off in your breath. Poor oral hygiene leading to gum disease adds to halitosis. Smoking and tobacco chewing can cause it as well. Finally medical conditions such as diabetes, liver and kidney disease can cause halitosis.
