Many options: Obviously you want this treated. Get with a couple of folks including an endocriologist or two, and ask about radioactive iodine, suppression with exogenous thyroid. Then make your own decision based on all the information.

See the doctor: You're obviously way overdue to see the doctor, and surgery is almost certainly indicated. It could have been prevented if caught earlier and treated appropriately.

Obstruction: Neck mass, regardless of the organ or cause, at a certain size (also regardless of delay) need relief of obstruction (airway/feeding tube). Primary concern is avoid loss of airway. Surgery might be the best choice if you are obstructed. However, most of the thyroid related diseases require both medical and surgical therapies. Be prepared for both. Take family with you to your doc visits. Good luck.
