Different route: This is an issue where just using stronger versions of the same tool (opiates) doesn't necessarily work. It sounds like you may need another route of therapy; if you have sciatica, your doctor should consider an agent for neuropathic pain, such as nortriptyline, gabapentin, or pregabalin.

Fentanyl Patch: A safe and effective method of pain relief can be from transdermal patches. You put them on once every three days and you can forget about taking pills. Fentanyl is 100x stronger than morphine. This is a heavy heavy duty opiate. However if used as prescribed it can be better than just popping pills. You most likely will need a specialist to rx it for you.

Opiates: are not the answer for your pain. You may need interventional therapies and/or surgery. If you've tried all pain meds forget the pain meds they will not work. See a true spine specialist to determine your actual treatment.
