Get a second opinion: Sciatica refers to pain from pressure on nerves as they leave the lumbar spine. The pain can be felt all the way down the leg. Causes include herniated discs. In addition to Naproxen type medication other medication to stop nerve pain can be helpfull such as gabapentin or Lyrica (pregabalin). Sometimes a brief course of oral corticosteroids help. Epidural corticosteroids probably don't help.

More than pain!: I understand that you'd like pain relief - but you need more than that. Loss of reflex is nothing to ignore. I see you've had evaluations, but you still need closure. Either they need to keep looking or trying to relieve your pain or they need to convince you that there's nothing to be done. It's hard - but if you can, consider alternatives like accupuncture, etc. Try till you win or run out.

Cause???: you have to find out what is causing the damage to your S1 nerve. your wasting your time with just symptomatic therapy, have to get to the root of the cause. pinched nerve due to disk? periformis syndrome?? neuropathy??
