If you are sensitive: Caffeine, in patients sensitive to it, may have high blood pressure or irregular heart rhythms as a result of the caffeine. It may also cause jitteriness that may have psychosocial implications. If a short term issue, there may be no long term problems, but chronic use, especially if combined with other suboptimal lifestyle choices like alcohol abuse, can lead to structural damage if sensitive.
