I would start: By using a an antifungal cream like lotrimin, (clotrimazole) mix with Hydrocortisone and possibly even some topical silver cream. Those three in combo, used every day for a month then 2 d per week should help. The pigmentation could be do to scratching but if overweight and/or strong family hx of type 2 dm, look up acanthosis nigricans. Good luck.
