MVR: Mvr could be repair or replacement that's why we try to avoid abbreviations if repair, may be a few months. Depending on replacement choice could be 3 months or if a mechanical valve could be for life!

Need higher INR: If your mvr is a mechanical valve, you'll need systemic anticoagulation forever. Pradaxa and xeralto are newer anticoagulants like warfarin, but no pt/inr is necessary. Also, for a mechanical valve, the goal inr is 2.5-3.5.

Indefinitely: For a mechanical valve replacement, warfarin will be needed indefinitely with close follow up of the inr (target should be 2.5-3.5 for most of the newer mechanical valves). Vitamin k containing foods (like leafy greens) have to be minimized to avoid neutralizing the warfarin effect. The newer anticoagulants (pradaxa/xarelto) are not approved for this indication w cases of valve thrombosis report.
