Yes and yes: There are various procedures for gerd, the most common being a nissen fundoplication, where part of the stomach (fundus) is wrapped fully around the lower esophagus. Long term it is usually fine, but could slip or become unwrapped. There could also be reflux through an intact wrap. There should have been a battery of tests before operation, and these can be repeated to see what's wrong if anything.
