Treatment is Medical: Its hard to answer this question without seeing the images as there are many things that could cause her pain, with fracture and arthritis being the most common in an 88 year old. If the MRI is truly normal then treatment of the pain with muscle relaxants and pain medication is an option. "back strain " is a clinical diagnosis, not one made on an mri.

Many ways: Is there a compression fracture? Treatment depends on what is source of the pain. If muscle strain, then heat, muscle relaxants and physical therapy. Anti inflammatories can be tried with caution. Topical Lidocaine patches may help. If your doctor can't help reach out to a pain specilist or geriatrician.
