Lots of things: He needs to see his doctor. Particularly if he is very thirsty and urinates a lot, he could have diabet mellitus. Diabetes insipidus may also need to be addressed and involves the pituitary gland or kidneys. Incontinence could certainly be neurological but it makes me think more of seizures (not likely to be conscious when happening) or nerve root compression. Severe constipation may also do this.
