Many possibilities: Call his oncologist. Reflux, airway compression, etc. Just because he is not actively being treated, it doesn't mean he can have his symptoms evaluated. I am sure his oncologist would be happy to see him and try and help him out.

Stage 4 lung ca: Most likley has heartburn, but needs to have chest x-ray to make sure he does not have pleural effusion( fluid in chest).
