Topirimate was upped from 75mg to 200mg/day for hemiplegic migraines--but now i can't focus or find words i want to say, etc...just feel exhausted 24/7?

Not Unusual: What you are experiencing is not unusual. Dose reduction or switching to another med in that class of drugs may reduce this problem for you. If the topirimate did help with the headache, switching may make sense.

Common: Topirimate inhibits ampa glutamate, which involves critical pathway for speech and recent memory. That's why we call it dope-a-max! although dosage related, best to switch to depakote, naproxen, or even otc agents such as butterbur, feverfew, riboflavin, coq-10, as these all can work better without the cognitive issues.
