Psa: New aua guidelines indicate psa above 2.5 may indicate a need for a biopsy. In a 60 yo with a family history and a psa of 3.1 i would advise bx if pt has >10 year life expectany, if no family history i would be less adamant...But i would offer bx or repeat psa in 3-6 months. Clearly dre and older psa results would be important.
