A few: Maybe, there is a foreign body in the heel, maybe a habitual toe walker, maybe a contracted achilles' tendon, there could also be a problem with the hip. You should take him to the doctor.

See below: He may have injured it by stepping on something. If it does not get better take him to the doctor.
