Many things: You describe a number of conditions from simple dermatitis to infection to an arteriovenous malformation. This needs to be evaluated. X-rays are needed as well.

Many things: It could be something as simple as irritation from shoes. It could also be a dermatitis, athlete's foot, a circulation issue, etc. There are too many possibilities to diagnose without a proper examination. If it persists, see a podiatrist.
