Obturator entrapment: Yes open release is carried out more commonly. In your area i would seek out a neurosurgeon or sports medicine specialist familiar with the diagnosis and treatment.

Guided injection: I would try an injection around the nerve with ultrasound guidance prior to surgery. Also make sure you have had emg/ncs to see if there is true nerve damage.
