Need more info: Mesh infections typically present with skin changes at the site of repair. If the overlying wound is perfectly healed, it's unlikely. Ct scans can sometimes detect fluid around the mesh which would suggest infection. Most mesh infections are with staph epidermidis which is a slow-indolent bacteria. I am assuming the neg ct didn't show a recurrence which could account for the pain you have.

Probably: Need nuclear medicine white blood cell scan to see if infection likely. Biopsy also can be helpful. Can also be foreign body reaction, which can depend on type of mesh used. Test for unusual infections such as candida, aspergillosis, mycobacterium and mrsa.

Doubtful: After 5 years it is less likely to be infection, but still possible. It could also be an infected suture if permanent sutures were used. If its bad enough, you may consider reoperation, but i would reserve this for severe symptoms, especially in light of a normal ct scan. Make sure the surgeon reviews the ct can however, the radiologist may not see everything.
