Yes: An endodontist can do chairside tests to see if you have a necrotic pulp causing the problem. If the lesion is not of endodontic origin, you will be referred to an oral surgeon for exam and diagnosis.

Yes: You may want to start with your general dentist. He may perform additional testing or refer you to a specialist. Even thought a tooth is not hurting you, does not mean that there is not a problem.

Re-image: You're overdue to have the area reimagined to see if there are still signed of bone loss. Images should be compared for size & shape of the lesion. Would suggest examination by a Endodontist. Radiolucency indicates bone loss and may signify infection which can spread to other organ systems.
