Apprx. 4 weeks and i went for vaginal ultrasound yesterday but they didn't see anything other than thickened uterine wall what could this mean?

Probably normal: 4 weeks is usually not quite far enough along to see anything with ultrasound, even with the vaginal transducer. If there is an intrauterine pregnancy, the first thing to show up will be a gestational sac, which usually becomes visible by 4.5 - 5 weeks. Be patient and don't worry at this point.
