Doing surgery and not doing surgery both have implications and more testing will likely be needed to figure out how to proceed. But you will definitely need medical and/or surgical treatment of some sort.

The presence of : Endometriomas suggests a sever form of endometriosis. Your body created an immune response around the endometriotic implants on the ovary and basically walled itself off. Problem is it conitnues to bleed and with time expands and can cause pain. Surgery is the only way to remove them however this depends on your reproductive goals and their size and the severity of your pain.
