Need more info: There is no surgery for tinnitus unless the tinnitus is caused by otitis media (fluid behind the ear drum). Other than an exam under a microscope, the best way to check is with a tympanogram. The tympanogram should not be inconclusive - fluid should register as a type b. If you have a type b tympanogram with fluid behind the ear drum, then a myringtomomy may help.

Not necessarily: Not necessarily. Effusions can be treated medically and could cause ringing. Sometimes surgery is indicated to help get rid of the effusion. A full exam with an audiogram would be indicated for further work up and appropriate diagnosis.

Most likely not: Fluid is usually pretty obvious. Drainage of fluid improves hearing and if the tinnitus is due to fluid it usually goes away. If the ear with tinnitus hasn't also lost a significant amount of hearing then fluid is unlikely.
